For your convenience we are providing the following forms for you to review, print and complete before your visit. These forms are also available to you at each of our office locations. You only need to complete the forms if you haven't seen an ENT physician in over a year.

Please remember to also bring:
- Your insurance card.
- Referral form(s), if required by your insurance company.
- Copies of any x-ray films associated with your problem.
- Please be prepared to pay your co-payment prior to your appointment if applicable.
Insurance
We accept most insurance. If you have a Health Maintenance Organization plan (HMO), it is your responsibility to obtain the referral from your primary care physician's office prior to your appointment.
Please contact your insurance carrier to determine your specific benefits including any deductibles, co-insurance, and out-of-pocket expenses. Please verify that our physicians are in-network providers on your plan.
Our office staff will be happy to assist if you have question.
Payment Information
Our physicians are providers for many insurance plans. If we are contracted with your insurance carrier, we will file an insurance claim for the insurance portion of your office visit(s), and make the necessary contractual adjustments. Co-payments and payment for services not covered by your insurance will be due at your visit. For your convenience, we accept cash, check or all major credit cards. We will also file your claims for all surgical procedures. Remaining balances and deductibles will be billed to you. |
